Procedure Checklist

Have you completed the Registration/Waiver and Annual Information Forms? Make sure they are filled out completely and signed on the back side.  You can download them by clicking on the links above.

Do you have a completed Annual Information Form on file with FVSRA? Call the FVSRA office or visit our web site to obtain one. All participants must have a completed AIF on file. Please notify FVSRA of any changes of AIF information during the course of the year.

Will medication need to be dispensed at a program(s)? Please indicate so on the registration form and staff will call you.

Have you included payment in full for all programs? Any outstanding balances from previous seasons must be paid before this registration will be valid.

Please make sure your AIF is up-to-date. Bring or mail the registration form, AIF and payment to the FVSRA office.

Registration confirmations will be mailed.

A $20 fee will be charged to the issuer of any returned checks.

Liability Waiver
The Association’s liability insurance carrier requires that prior to participation in any of the FVSRA programs, the legal authority must sign the waiver. If a participant 18 years or older is able to sign contracts to legally enter into agreements, then he/she should sign the waiver. If not, the parent or legal guardian must do so.

PARTICIPATION WILL BE DENIED if the signature of adult participant or parent/guardian, and date are not on Waiver and Release of All Claims.
